These are stylised examples. Real companies, real shapes - invented details. They show how the same record covers everything from a £4,820 refund to a brain implant reverting to safe mode. Each example is the same shape: who said yes, what they knew, what they expected, the gate between deciding and acting, and the hash seal so you can see if the record was changed later. The difference is the consequence.
